If you want your records there is a $128.50 fee for an individual plus a document to sign. The fine print on the back side under Miscellaneous Personal Information it says “DOCUdavit is entitled to conduct a personal investigation or credit check upon the patient, subject to applicable legislation.”. Why would they need to investigate a patient or run a credit check? Doctors offices are increasingly using Apps for appointment scheduling and people are having great difficulty getting appointments with their doctors through this method.
